If you have any questions about our office policies, please call our office during business hours.


If you are a new patient, please arrive at our office 30 to 15 minutes prior to your scheduled appointment.

Broken Appointments
Dr. Smith allows a specific amount of time reserved only for you and we strongly encourage all patients to keep their appointments. If you must change your appointment, we ask that you notify us at least 24 hours prior for single appointments and 48 hours prior for multiple family appointments. This will allow us to accommodate another patient who needs to be seen. We appreciate your understanding.

For patients with dental insurance, we are happy to work with your carrier in order to maximize your benefit coverage. Dr. Smith is a preferred provider for the following companies:

For this reason, we work hard to provide a variety of payment options to help you receive the dental care you need and deserve with respect to your budget. We are always available to answer your questions or assist you in any way we can. Forms of payment acceptable are cash, check, Visa, MasterCard and Discover. Also, we offer 6 month No Interest and Extended Payment Plans through CareCredit.

  • 10% discount for Seniors 65 plus*
  • Multiple family member discount*
  • 10% discount for treatment of $300 plus when paid at the time of the appointment*

*Discounts can not be combined therefore they are only applicable to patients without insurance.
